Copy of Transcript (48 Semester College Units, Associate Degree or higher or NCLB exam)
CPR/First Aid Certification (Must have valid CPR & First Aid )
Letter of Introduction
Letter(s) of Recommendation (Must Submit Two (2) Letters Dated within the last Twelve (12) months.)
Proof of HS Graduation (Or equivalent)
Resume
