Compensation Range

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities

GENERAL PURPOSE

The Senior Social Worker will facilitate the ongoing development and implementation of a Children’s Health Home Care Management program and provide leadership to a team of Children’s Health Home Team Leads and Care Managers for the purpose of implementing the Golisano Children’s Hospital at Strong Health Home Care Management Program. The Senior Social Worker will assure that comprehensive care management services are provided to enrolled patients in accordance with established NYSDOH Children’s Health Home requirements, GCH@SHH protocols, Hospital and Social Work Division policies and procedures. The Senior Social Worker has primary responsibility for the recruitment, training, supervision and evaluation of the Children’s Health Home Team Leads and oversight of the Care Managers. The Senior Social Worker facilitates the integration of the Children’s Health Home program services with the GCH@S general pediatric practice, sub-specialty practices, and child and adolescent behavioral health practices. The Senior Social Worker develops strong, collaborative relationships with a broad array of community based formal and informal programs and services that benefit the patient population. The Senior Social Worker performs quality assurance activities related to the program, program evaluation and reports as indicated.

Essential Functions

  • Responsible for recruitment, and training of CHH clinical staff and supervision of CHH team leads.
  • Assures that staff have received all background checks and fingerprint requirements.
  • Promotes reflective practice, cultural sensitivity, equity, diversity, inclusion, ICARE values, patient centered care and ongoing professional development.
  • Assures that all mandatory trainings including certification in the CANS – NY assessment and health screenings occur.
  • Works collaboratively with Quality Manager and Team Leads in auditing for compliance with CHHUNY expectations and billable standards.
  • Completes competency assessments and performance evaluations with input from members of the care team.
  • Applies personnel management actions if needed.
  • Follows compliance protocols and other policies governing CHH CMAs.
  • Utilizes recognition and is responsive to staff satisfaction issues.
  • Creates on – call schedule.
  • Reviews time sheets for accuracy.
  • Reviews CHH referrals for appropriateness responding back to CHUNNY regarding those not appropriate. and assigns accepted cases to CHH care managers, monitoring for recommended case load size and mix of acuity.
  • Reassigns cases upon care manager departures or long-term disability.
  • When required assist in the process of transferring cases to another care management agency.
  • Facilitates and participates in CHHUNY and NYSDOH trainings and meetings and disseminates information to staff.
  • Identifies and arranges for additional training in areas in need of improvement.
  • Participates in Social Work Division and GCH@S CHH meetings in a leadership role.
  • Coordinates the Children’s Health Home activities with those of GCH@S discharge coordinators, social workers, PCP’s, specialty services, CPS and community service providers.
  • Develops collaborative relationships with community providers to enhance the availability of community resources and patient outcomes.
  • Serves on collaboratives that relate to the GCH@S CHH.

Other duties as assigned

Minimum Education & Experience

  • The Senior Social Worker requires 2 years of professional social work practice and must have an MSW degree from an accredited school of social work. Required
  • Supervisory and program evaluation experience is preferred
  • Knowledge of health care and Health Home delivery systems Preferred

Knowledge, Skills And Abilities

  • The Senior Social Worker must have significant clinical experience with a pediatric population in either in a medical, behavioral health or child welfare setting. Required

LICENSES AND CERTIFICATIONS

  • Licensed MSW in New York State Required
