Job Summary

The HHA, provides personal care as planned, directed and supervised by an RN to patients in their homes and is responsible for making observations and reports regarding the personal care needs, including the physical, psychosocial and environmental needs of patients and their families. The home health aide (HHA) provides home health services to patients in the home meeting all agency, state and federal regulations.

As a member of a multi-disciplinary home health staff, the HHA function as a team member for assigned individuals and families and ensures the provision of safe delivery of personal care following all department policies and state and federal regulations.

Responsibilities*

Essential

  • Provides hands-on personal care as planned and supervised by the RN and/or case manager.
  • Performs simple procedures as an extension of therapy or nursing services in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
  • Assists in ambulation or exercises in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
  • Assists in administering medications that are ordinarily self-administered in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
  • Performs and documents observations and interventions in the home according to the established Plan of Care and according to all department policies and state and federal regulations.
  • Works with the Customer Service Representatives and Nursing Supervisors/Clinical Managers to schedule work time and plan assignments.
  • Participates as an interdisciplinary team member by reporting changes in the patient's condition to a registered nurse or other appropriate skilled professional and completing pertinent records in compliance with the organization's policies and procedures.
  • Communicates and interacts with patient/caregivers and families in a positive and effective manner.
  • Encourages and assists patients and caregivers to achieve their maximum degree of independence.
  • Participates in department development by sharing observations about individual and aggregate health care needs, department reputation, program planning needs, etc. to administrative staff of the department.
  • Manages assigned patient caseload, meeting department productivity expectations.
  • Provides care based on the Plan of Care developed by the RN, department mission, protocols and Patient's Bill of Rights.
  • Completes all documentation according to department guidelines to ensure inclusion in the medical record within regulated time frames.
  • Participates in the Quality Assessment and Performance Improvement (QAPI) program.
  • Participates in organization sponsored in-service training.
  • Patient and Family Centered Care:
  • Actively engages the patient, patient representative (if any), and family by exhibiting respect and dignity, sharing information in a timely and unbiased manner, and welcoming and encouraging participation to achieve shared decision making.
  • Collaborates with other members of the team, including the patient, patient representative (if any) and family, to achieve the ideal patient care experience.
  • Communicates and interacts respectfully and appropriately with coworkers and supervisors.


Required Qualifications*

  • High school diploma or equivalent
  • Prior experience as a health aide, nursing assistant or medical assistant
  • Ability to read, write and carry out directions
  • Ability to demonstrate compassionate attitude toward care of the sick
  • Ability to travel from point A to point B throughout the work day as assigned
  • Successful completion of the pre-employment screening test


Physical Requirements

The home health aide will be required to carry out assignments in patients' homes, health care institutions and HCS offices. The employee will perform patient care duties at the clients' homes during the work period. These duties include bathing activities, assisting and/or performing prescribed exercises, assisting with ambulating and transferring clients from one place to another, ADL assist, etc. The duties require the ability to lift up to 50 lbs on a regular basis and the ability to maintain stable balance while performing job duties, especially during tasks associated with patient ambulation and transfers. The duties also require the ability to reach, bend, push, pull, climb stairs, drive, and tolerate prolonged or considerable walking distances and/or standing for a great length of time. Must be able to walk on uneven surfaces in all weather conditions considering varying work site locations. Visual acuity and hearing is required to perform clinical tasks and skills. Employee may drive personal or University- owned vehicle to various work site locations on a regular basis including in-town and highway driving in good and inclement weather.

Desired Qualifications*

  • Certified nursing assistant (Completion of a nurse aide program consisting of 75 hours of training and competency program)
  • Prior home care experience with completion of home health agency nurse aide competency program
  • One year of education from an accredited nursing program
  • Excellent customer service skills
  • Proficiency in email, communication and web based learning

Modes of Work

Positions that are eligible for hybrid or mobile/remote work mode are at the discretion of the hiring department. Work agreements are reviewed annually at a minimum and are subject to change at any time, and for any reason, throughout the course of employment. Learn more about the work modes.

Work Schedule

Day shifts, rotating weekends and holidays.

Union Affiliation

This position is covered under the collective bargaining agreement between the U-M and the AFSCME union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third-party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.

In addition to the screenings indicated above under Michigan law, a criminal history check including fingerprinting is required as a condition of transfer or employment for this position.
