UConn Health is searching for a Heart Failure clinical Program Coordinator. This position is responsible for coordinating the daily operations of an acute clinical program across the entire continuum of care. Works collaboratively with patients, their families, health care providers, and community groups throughout the continuum of care. Serves as a leader for the program's performance improvement initiatives and community outreach.

SCHEDULE: Full time, 40 hours per week Monday - Friday, 8:00 a.m. to 4:30 p.m. flexibility needed to work other hours, 30-minute unpaid meal break.

SUPERVISION RECEIVED : Works under limited supervision of a Director, Nursing or other designee.

SUPERVISION EXERCISED : Clinical Program Team Members

EXAMPLES OF DUTIES :

• Creates and establishes protocols, policies, forms, and tools related to the American Heart Association's Get With The Guidelines ® - Heart Failure quality improvement program

• Monitors accreditation policies and procedures, responsible for the facilitation of the program for possible advanced certification.

• Analyze and evaluate patient encounters, logs, and data on the program.

• Collects measurement data related to and evaluates processes and/or outcomes of care, and collaborates with the Cardiology Lead APRN and Medical Director of Cardiology to address deficiencies.

• Audits and/or maintains the applicable heart failure database (ex. AHA GWTG Heart Failure Registry).

• Chairs, co-chairs or facilities various committee meetings related to program care.

• Supports the education and training of the medical and clinical staff, including orientation and continuing in-service- programs for staff impacting the program.

• Actively participates in correcting deficiencies in care and coordinating performance improvement peer review through multi-disciplinary subcommittee meetings.

• Organizes community outreach activities/events related to the program as appropriate.

• Collaborates with marketing regarding program and outreach and serves as community liaison on behalf of the program and hospital.

• Obtains and maintains program certification/designation.

• Performs other related duties as required

MINIMUM QUALIFICATIONS REQUIRED:

KNOWLEDGE, SKILLS AND ABILITIES :

• Knowledge of the principles of program management; knowledge of specialty anatomy and physiology, diagnosis, identification and triage, preventive care, hyperacute care, diagnostics, acute care, post-acute care and discharge planning, rehabilitation, systems and quality, community care.

• Role model in critical thinking, assessment skills and evidenced based nursing care.

• Effective communication [oral and written] and interpersonal skills

• Considerable analytical skills

• Proficiency in use of Microsoft Office application: Excel, Publisher, Word and PowerPoint

• Strong leadership skills, time and project management skills

• Ability to lead educational activities

• Ability to facilitate change, work harmoniously with teams and manage effectively

• Ability to multitask

GENERAL EXPERIENCE & TRAINING:

• Bachelor's degree in Nursing

• A minimum of three [3] years clinical experience in a critical care or step-down level unit and must have cared for patients with heart failure.

• Must possess and maintain Connecticut State license during employment, in the required discipline.

• Must hold and maintain current Advanced Cardiac Life Support (ACLS)

PREFERRED QUALIFICATIONS

• Master's Degree in Nursing or related field (ex. Education or nursing leadership)

WORKING CONDITIONS:

• Must be able to lift up to 50 lbs and push and pull heavy objects.

• Be prepared to sit or stand for extended periods of time.

• Works irregular hours.

• May be exposed to communicable/infectious diseases.
