Job DescriptionUnder general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory Pediatric Heart Center Nurse works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. S/he will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Nurse Care Coordinator will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. In addition, the Clinical Nurse Care Coordinator will assist in the development of patient care standards and practices.Â
The Pediatric Heart Center Nurse Care Coordinator is responsible for coordinating outpatient services and needs for a panel of pediatric cardiology patients and providers in the ambulatory setting. His/her responsibilities include both in clinic procedures, managing the Outpatient Heart Center clinic flow as well as assisting with insurance authorization and prior medication authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care. This position is 100% in-person onsite. This position support patient care primarily in San Francisco and as needed in Oakland and satellite clinics. Â
Qualifications Required Qualifications: - Bachelorâs Degree in Nursing (BSN) - Five or more years of pediatric cardiac inpatient nursing experience - Experience working in an out-patient/ambulatory setting - Knowledge of Pediatric Congenital Heart Disease - Strong heart rhythm interpretation experience - Strong PIV insertion skills - Experience performing Bubble studies - This position requires flexibilityâ¯to orient and work at all UCSF Medical Center locations.
