Job Summary

The Wraparound Hospital-based Violence Intervention Program is under the UC Davis Medical Center, Patient Care Services Department. The role of this position is to assess community and resource support systems for participants of the Wraparound program and their families; formulate patient-centered care plans; assist Wraparound patients and their families with navigation of health system and community resources; and arrange and monitor the provision of material supportive services for post-discharge holistic recovery from violent injury.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and / or certification if required of the position

  • Bachelor's degree in related area and / or equivalent work-related experience
  • Completion of CITI Program for Social and Behavioral Researcher (SBE) within 3 months of hire
  • Completion of Mental Health First Aid training within 6 months of hire
  • Experience working with youth and young adults affected by violence or involved with the Justice System.
  • Experience with direct service provision or navigation assistance for individuals in the health care, community health, or social services settings.
  • Knowledge of intervention techniques and trauma-informed practices.
  • Knowledge of Patient Rights & Responsibilities.
  • Strong interpersonal and customer service skills. Ability to communicate and resolve issues effectively with a diverse population of patients, staff and physicians.
  • Excellent analytical and problem-solving skills. Ability to develop solutions and recommend changes and follow through with implementation.
  • Excellent written and verbal communication skills.
  • Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area.

Preferred Qualifications

  • Violence Prevention Professional (VPP) Certification
  • Experience working with Hospital-based Violence Intervention Programs (HVIP)
  • Experience in health care or hospital settings
  • Experience working with community based service organizations

  • Department Overview

    The Trauma Prevention Program, within the UC Davis Medical Center Patient Care Services department, is responsible for fulfilling injury prevention requirements for American College of Surgeons Committee on Trauma (ACS COT) verification for Level I Pediatric and Level I Adult trauma centers. The mission of the Trauma Prevention Program is to prevent and reduce the leading causes of injury within the UCDH trauma center catchment area through education and outreach.
