What are the responsibilities and job description for the Vice President, Care Transitions position at University Health - San Antonio?
POSITION SUMMARY/RESPONSIBILITIES
The Vice President, Care Transitions will collaborate with other clinical leaders in the organization to support the engagement and success of care transition programs (In-patient Case Management, Ambulatory Coordination of Care, Nurse Telephone Triage, and the Command Center), outcomes, and policies. Acts in the capacity of an Officer of University Health. Demonstrates a strong commitment to the mission of University Health as well as to the proper stewardship of its resources.
EDUCATION/EXPERIENCE
Masters’ degree in Health Care Administration or related field is required. Bachelor’s degree and a minimum of ten years related experience may be substituted for the required education. Four years in health care management/administration is required. Aforementioned experience gained in a university-affiliated hospital is preferred.
