What are the responsibilities and job description for the Licensed Practical Nurse position at Universal Primary Care?
ESSENTIAL FUNCTIONS
- Assist providers with outpatient family medicine and obstetric patient care.
- Room patients, obtain vital signs, document medical histories, and prepare patients for examinations and procedures.
- Perform clinical tasks including immunizations, injections, point-of-care testing, EKGs, specimen collection, and medication administration within scope of practice.
- Support prenatal and postpartum visits, newborn care, and patient education.
- Coordinate referrals, diagnostic testing, and follow-up care.
- Communicate with patients regarding test results, medication questions, and care plans.
- Collaborate closely with physicians, advanced practice providers, medical assistants, patient service representatives, and the care management team to deliver exceptional patient-centered care.
- Demonstrates accountability for the provision of nursing care
- Assists with provider documents (“folder manager”)
- Collects data and communicates patient information in order to optimize patient outcomes
- Respects cultural customs and beliefs
- Assists in determining the health status and health needs of the patient and assists in the formulation of goals/outcomes
- Demonstrates the knowledge and skill necessary to provide care appropriate to the age and developmental level of the patient
- Assists in the coordination of human and material resources for the provision of care to assigned patients
- Participates with other members of the health care team for patient care delivery
- Demonstrates professional growth as evidenced by a minimum of ten(10) hours of related education annually
- Responsible for working with providers to best efficiently meet their needs and our patients’ needs
- Assists with procedures as appropriate and needed
- Provides stocking, storage, cleaning, communication nurse duties and others as assigned
- Responsible for NYSDOH Rx EMR paper
- Ensures safety of all medicines and medical supplies maintained at the practice site
EDUCATION/EXPERIENCE:
- Graduate of an accredited school of Practical Nursing
- Current New York state, LPN registration or eligible for licensure required
- Current Pennsylvania LPN license preferred
- Successful completion of BLS course required
- This position requires experience in all areas of nursing (primary care, OB, etc.)
- Excellent communication and organizational skills.
- Ability to thrive in a fast-paced outpatient setting.
- Passion for providing compassionate, high-quality care to patients and families.
Universal Primary Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our outpatient clinics in Salamanca and Olean, NY. In this role, you'll work alongside a Family Medicine with Obstetrics physician providing comprehensive care to patients of all ages, including prenatal, postpartum, newborn, pediatric, adult, and geriatric care.
