LPN

We are currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Long Term Care team. In this role, you will provide direct nursing care to residents, help coordinate daily care routines, and ensure a safe and supportive environment for individuals with varying healthcare needs.

Key Responsibilities:

• Deliver safe, direct nursing care to residents under the supervision of a Registered Nurse.
• Monitor resident health and report any changes or concerns to the supervising nurse or physician.
• Administer prescribed medications and treatments in accordance with approved nursing techniques.
• Assist with ADLs (Activities of Daily Living) and support restorative care efforts.
• Maintain accurate and timely documentation of resident care, progress notes, and treatment plans.
• Collaborate with interdisciplinary teams to develop and follow individualized care plans.
• Ensure compliance with all federal, state, and facility regulations and guidelines.
• Provide emotional support and establish a compassionate environment for residents and their families.

Requirements:

• Valid LPN license in the state of NY.
• Minimum of 1 year experience in long-term care, skilled nursing, or a similar healthcare setting preferred.
• Current BLS certification.
• Strong communication and organizational skills.
• Ability to work independently and as part of a team in a fast-paced environment.
