Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings, including long-term care, assisted living, and acute care environments. As an LPN/LVN, you will provide essential nursing services, support patient well-being, and collaborate with multidisciplinary teams to deliver high-quality care. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems.
- Administer medications and injections accurately, including IV infusions and catheter care, following established protocols and safety guidelines.
- Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
- Provide wound care, including suturing and dressing changes, while adhering to aseptic techniques and infection control policies.
- Support patients with activities of daily living, dementia care, Alzheimer's care, and memory care needs within geriatric or specialized settings.
- Monitor patients' responses to treatments, manage acute pain, and escalate concerns promptly to supervising physicians or registered nurses.
- Document all patient interactions thoroughly in medical records while maintaining strict confidentiality in accordance with HIPAA regulations.
- Collaborate effectively with physicians, case managers, caregivers, and families to ensure comprehensive patient-centered care.
Requirements
- Valid state licensure as an LPN or LVN with current certification to practice nursing in the respective jurisdiction.
- Proven experience working with diverse populations such as seniors, individuals with developmental disabilities, or those requiring hospice or palliative care.
- Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation and patient management.
- Knowledge of advanced nursing procedures including IV insertion, ventilator management, tube feeding, and catheterization.
- Experience in various clinical settings such as hospitals (including ICU or trauma centers), nursing homes, skilled nursing facilities, outpatient clinics, or home & community care environments.
- Strong understanding of medical terminology, ICD coding (ICD-9/ICD-10), and long-term care regulations.
- Excellent communication skills for effective caregiver communication and client interaction across multidisciplinary teams.
- Ability to prioritize tasks efficiently in fast-paced environments like emergency rooms or urgent care clinics while maintaining high standards of clinical confidentiality policies.
