The Certified Occupational Therapy Assistant (COTA) - Home Health provides compassionate, hands-on care to a diverse community-based caseload under the supervision of a licensed Occupational Therapist. In this role, the COTA applies clinical expertise and evidence-based best practices to implement individualized treatment plans, promote functional independence, and enhance quality of life while maintaining high standards for patient safety, satisfaction, and measurable outcomes.

Responsibilities
- Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.
- Consistently utilizes a patient-centered approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.
- Treats patients/clients to relieve pain, develop or restore function and maintain maximum performance using specified occupational therapy procedures.
- Directs and aids patients/clients in plan of treatment.
- Delivers quality care consistent with Home Health Conditions of Participation, qualifying home care criteria, payor coverage criteria, and providing visits/treatments consistent with the plan of care.
Education:
- Graduate of State Board approved program for Occupational Therapy Assistants
- Licensed, registered, or certified as an Occupational Therapy Assistant by the state(s) where providing services
- ICM Certification
- Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
- Mandatory Reporter: Child & Dependent Adult Abuse.
- CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
- Minimum CNA 75 hour
- Use of usual and customary equipment used to perform essential functions of the position.
