Partial Care Mental Health/Substance Abuse/Addictions Facility seeking compassionate Primary Care Coordinator who enjoys the challenge of working with Mental Health population.
Education:
- Bachelor’s Degree
Skills/Qualifications:
- 2-5 years of Human Services experience required
- Utilizing treatment team model
- Excellent documentation skills
- Responsibilities include treatment planning and all other required documentation
- Case Management, crisis intervention & facilitating psycho/education groups
- Provide exceptional individualized/group services
