Role Description The Veterans Case Manager is a full-time, hybrid role based in Miami, FL, with a combination of in-office and work-from-home responsibilities. This role provides comprehensive case management services to veterans and their families, including intake, assessment, service planning, referrals, and ongoing follow-up to support stability and well-being. The Veterans Case Manager coordinates access to housing, employment, healthcare, and social services, and maintains strong relationships with community partners, government agencies, and veteran-serving organizations. Daily tasks include meeting with clients, documenting case notes, tracking outcomes in case management systems, and preparing reports to support program evaluation and compliance. The role also involves outreach, participation in community events, and collaboration with internal teams to ensure veterans receive integrated, person-centered support.
Qualifications
- Strong case management, client advocacy, and service coordination skills, with the ability to develop and monitor individualized service plans.
- Excellent communication, interpersonal, and active listening skills to build trust and rapport with veterans and diverse community stakeholders.
- Organizational, time management, and documentation skills to maintain accurate records, track outcomes, and manage multiple cases effectively.
