Location: 140 North Frontage Road, Mansfield Center, CT 06250
Located at our new state-of-the-art Windham Regional Health and Wellness Center at 140 North Frontage Road, Mansfield Center, CT.

Case Manager - Community Support Services Description:
Provide and coordinate supportive services for clients with serious and prolonged mental illness.
As a case manager, you have a unique opportunity to forge a therapeutic connection with an adult who has been diagnosed with a mental health disorder. When individuals are asked what made the difference in their lives, the most frequent response is that they had someone they worked with who gave them hope that they can have what everyone wants: safe and decent housing, access to quality mental health and medical care, meaningful personal relationships, productive activities including work, and a connection to the community in which they live.
Community Support Program is a multidisciplinary team providing case management, housing services, individual support and group counseling. If you have experience with mental health, substance use, community support services and an understanding of the psychosocial issues of this population, please consider becoming part of our team.
This is a job in which every day is different, where you have the independence and flexibility to structure your time but the support and back up of seasoned and professional supervisors as well as the opportunity to collaborate with members of our clinical and community support teams.
This position involves extensive work in the community. Use of agency vehicles as well as use of own car (mileage and reimbursement).
Minimum Requirements:
Minimum Requirements: Bachelor’s Degree in a relevant human services field OR Associate’s Degree in a relevant human services field plus two years of relevant experience required.
Schedule: Monday – Friday, 8:30 am – 5:00 pm with one evening until 7:00 pm
Salary/Hourly Range:
$19.00 – $21.50 per hour (depending on experience)
Bilingual (Spanish/English) rate of pay: $19.96 – $22.59 per hour (depending on experience)
