The United Presbyterian Home seeks an experienced MDS & Care Plan Coordinator to join our team!  The MDS & Care Plan Coordinator is responsible for working with the entire resident care team to ensure the delivery of high-quality care and services consistent with regulations and established clinical best practices. The MDS & Care Plan Coordinator collects, organizes and maintains all information for completion of the Minimum Data Set. They accurately completes MDS’s on a timely basis and ensures MDS’s matches the care plan and delivery of care given in the facility. This position works closely with the Assistant Director of Nursing, Nursing staff, Social Services Director and the Director of Nursing.
 

Key responsibilities include:

  • Oversight and direction of the MDS and Care Plan process within the structure of state and federal regulations to provide the highest quality of care for our residents.
  • Monitoring of resident assessment time frames (admit, skilled, quarterly, annual, annual, change in condition MDS) to ensure assessments and care plans are completed within established guidelines.
  • Monitoring of Medicare residents for appropriate timelines of assessments, level of care and appropriate PPS categories.
  • Assisting with screening and reviewing resident referrals with IDT if needed.
  • Determination of diagnosis on admission.
  • Assurance of timely and accurate assessment/reassessment of residents’ needs, development and the implementation of individualized plans of care (PASRR Level 2) and evaluation of resident response to the plan of care in compliance with applicable regulations.
  • Participation in daily Stand-Up Meeting, Medicare meetings and nursing leadership meetings (PEAKD/QA).
  • Assistance with therapy recommendations and updating care plans to reflect the new recommendations.
  • Updating resident care plans with new or updated GDR’s as well as changes for psychotropics utilized or discontinued.
  • Updating Point Click Care Dashboard with pertinent information to be shared with the care team.
  • Oversight of the Restorative Program
  • Assisting DON in monitoring and tracking resident falls, fall interventions and updating the resident care plans with those interventions.
  • Communicating with nursing personnel daily at all levels, providing clear, concise information and feedback as well as specific care plan changes.
  • Review of resident device list and updates it as needed to match each resident’s individual care plans.
  • Monthly tracking and submission of Monthly Ombudsman report.
  • Setting and updating bath schedule for residents.
  • Acceptance of assigned duties, instructions, and/or correction in a cooperative manner, voicing concerns or disagreement in a professional manner.
  • Assisting the Director of Nursing as needed in the hiring/termination protocols as needed. 
  • Assisting employees in a comprehensive orientation to the health center and campus.
  • Assumption of administrative responsibilities as delegated when the Director of Nursing is on vacation or otherwise unavailable
Qualifications:

  • Performance of other duties, including audits and rounds, and projects as assigned by supervisor.
  • Assistance in coordination of the leadership on-call schedule and participation in on-call rotation monthly.
  • Act as in-house expert regarding standards of practice, rules/regulations, professional code of ethics and serve as a role model for nursing and other facility personnel by following and enforcing facility policies and procedures. Adheres to facility policies and procedures including but not limited to infection control, safety, resident rights, chain of command, confidentiality, and employee handbook.
  • Ability to instill UP Homes mission, vision and values within each employee and create a workplace centered around resident centered care. 
  • Advanced preparation or experience in geriatrics and/or LTC regulations is preferred
  • Experience in leadership and/or management skills
  • May possess additional preferred certifications of CPR, Infection Control and/or DAA training.

Required education for this role includes:

  • Graduate of an accredited school of nursing
  • Possession of and responsibility for all actions necessary to maintain a current license as a Registered Nurse with the Iowa State Board of Nursing
  • MDS Certification is preferred or willingness to get certified
