Registered Nurse (RN) – Home Health / Wound Care

Locations: Gallup, NM and Grants, NM
Pay: Rate is negotiable based on experience and qualifications

We are looking for a compassionate and reliable Registered Nurse (RN) to provide skilled nursing care to clients in their homes. The RN will provide wound care, perform nursing assessments, follow physician orders, and coordinate care with the client’s healthcare team.

Responsibilities

• Provide skilled nursing care in the client's home
• Perform initial and ongoing nursing assessments
• Provide wound care and dressing changes
• Monitor wounds and report changes in the client's condition
• Follow physician orders and the client's care plan
• Follow physician-ordered laboratory tests and lab orders
• Monitor vital signs and overall health status
• Provide medication-related support and patient education as appropriate
• Educate clients and families on proper care, safety, and health management
• Report significant changes in the client's condition to the appropriate healthcare provider
• Maintain accurate and timely nursing documentation
• Coordinate care with physicians, caregivers, therapists, and other healthcare professionals
• Support clients with chronic conditions and ongoing health needs
• Follow infection control, HIPAA, and agency policies and procedures

Requirements

• Current and active New Mexico RN license
• CPR/BLS certification preferred
• Previous home health, wound care, or skilled nursing experience preferred
• Reliable transportation to travel to client homes
• Strong communication and documentation skills
• Ability to work independently and provide quality care in a home setting
