Key Responsibilities
- Administer medications and treatments as prescribed.
- Monitor residents’ health status and report changes to the RN or physician.
- Assist with personal care tasks including bathing, dressing, grooming, and toileting.
- Support residents with mobility and transfers using appropriate equipment.
- Maintain accurate documentation of care and observations.
- Collaborate with care staff to ensure individualized care plans are followed.
- Provide emotional support and behavioral redirection as needed.
- Accompany residents to medical appointments when required.
- Participate in resident care conferences and interdisciplinary team meetings.
