QUALIFICATIONS
- Bachelors in social work or human services required; master’s in social work preferred.
- 5 years of experience working in a social work setting required with the ability to demonstrate successful case management skills.
- Skilled in crisis management, trauma-informed care, documentation.
- Must be compassionate, empathetic, client-focused, and results-oriented.
- Ability to establish and maintain relationships with clients and community partners.
- History of working with diverse and challenging populations.
- Knowledge of substance abuse, mental health, and HIV a plus.
- Lived experience that allows the successful candidate to understand and relate to the program clients.
- Must be proficient in Microsoft Office suite products with ability to learn and use the required HMIS program.
- Valid Driver’s License.
PURPOSE AND FUNCTION
The Case Manager serves as an advocate for clients needing specialized services in housing stability; performs comprehensive and complete assessment intakes, provides linkage to community agencies, and client driven case management to targeted populations.
RESPONSIBILITIES

- Maintain confidentiality and client information.
- Case management of housing program clients
- Make appropriate referrals to other providers as well as other community agencies, as necessary, to minimize crisis and maximize self-sufficiency.
- Assist clients referred by other service providers with social service needs.
- Conduct periodic community meetings with housing program clients.
- Plan and engage clients in community activities.
- Document clients’ progress utilizing the HMIS system and prepare monthly reports.
- Keep client files current with all necessary documentation.
- Assist with the day-to-day operations of the program and office.
- Perform other duties as assigned by the Program Manager
