What are the responsibilities and job description for the Case Manager position at Family Promise of Beaufort County?
This is a Part-Time position.
Wednesday - Sunday 3pm - 8pm
The Shelter Case Manager is responsible for administering all aspects of the Shelter Program by assuring families receive the necessary tools to become self-sufficient and ultimately move into and remain in affordable, sustainable housing.
Duties:
- Serve as point of contact with families beginning with the intake interview through completion of the Shelter Program.
- Perform weekly meetings with each family to create and review goals/progress.
- Serve as point person for all childcare needs.
- Assure families receive all government services for which they qualify.
- Provide access, support, and resources to assist guests with employment searches.
- Assist with transporting clients to appointments, as needed.
- Maintain a full census and success rate of at least 80%.
- Complete case management data entry including HMIS, Charity Tracker, and Case Management databases.
- Develop and maintain relationships with local landlords and assist families with housing options.
- Serve as on-call Case Manager.
- Performs other reasonable responsibilities, as required.
Qualifications:
- Associate degree required in (social work or human services or similar preferred)
- Ability to communicate effectively with a diverse population.
- Handle multiple tasks simultaneously and reprioritize, as needed.
- Work independently with minimal direction.
- Flexibility to adapt to a fluid work environment.
- Strong problem solving skills.
- Good computer skills.
- Strong written and verbal skills.
