Role Description
The Assessment Coordinator plays a key role in assisting individuals beginning their recovery journey. This full-time, on-site position, based in Mesa, AZ, involves conducting comprehensive assessments, coordinating case management, and developing individualized treatment plans tailored to client needs. The coordinator will also collaborate with clinicians, families, and community resources to ensure a seamless process for treatment entry and continuity of care. Additional responsibilities include maintaining accurate documentation, scheduling assessments, and contributing to training and education efforts for staff and clients.
Qualifications
- Strong Communication and interpersonal skills to effectively collaborate with clients, families, and team members
- Case Management experience, including coordinating care and developing individualized treatment plans
- Background in conducting assessments
- Research skills to identify best practices and resources for client recovery plans
- Knowledge of behavioral health treatment modalities, including trauma-informed care and co-occurring disorder recovery
- Familiarity with documentation and compliance in a clinical setting
