Unfiltered Therapy LLC is seeking compassionate, motivated professionals to join our growing Comprehensive Community Services (CCS) team. We are currently hiring Physical Health Monitoring (PHM) and Individual Skill Development and Enhancement (ISDE) providers to support children, adolescents, and adults throughout Dane County and surrounding communities.

About CCS
Comprehensive Community Services (CCS) is a voluntary, recovery-focused Medicaid program designed to support individuals experiencing mental health and/or substance use challenges. Services are individualized and community-based, helping members achieve their recovery goals through skill development, education, advocacy, and coordination of care.
Position Summary
Providers meet individually with members in their homes and throughout the community to provide education, skill development, and recovery-oriented support. Services are individualized based on each member's treatment plan and recovery goals.
Providers have the flexibility to build their own schedules while working closely with Service Facilitators, therapists, psychiatrists, family members, and other treatment team members.
Primary ResponsibilitiesPhysical Health Monitoring (PHM)
Providers assist members in developing skills to effectively manage their physical health as it relates to their mental health and/or substance use recovery.
Responsibilities include:
- Teaching members how physical health impacts emotional wellness
- Providing education regarding medications and medication adherence
- Supporting healthy nutrition, hydration, sleep hygiene, and exercise routines
- Assisting members in understanding chronic health conditions
- Promoting preventative healthcare and wellness habits
- Encouraging attendance at medical appointments
- Supporting communication with healthcare providers
- Teaching members to recognize changes in physical health that may impact mental health
- Building independent health management skills
Individual Skill Development & Enhancement (ISDE)
Providers assist members in building practical life skills that increase independence and improve daily functioning.
Responsibilities include:
- Independent living skills
- Household organization and routines
- Budgeting and financial management
- Meal planning and grocery shopping
- Transportation and community navigation
- Time management and executive functioning
- Communication and interpersonal skills
- Emotional regulation and coping skills
- Problem-solving and decision-making
- Community engagement and social skill development
- Vocational readiness and employment support
- Building confidence and independence within the community
General Responsibilities
- Provide community-based services in homes and community settings
- Develop positive therapeutic relationships with members
- Support members in achieving individualized recovery goals
- Collaborate with interdisciplinary treatment teams
- Participate in monthly supervision and team meetings
- Attend ongoing trainings and professional development opportunities
- Complete accurate CCS documentation within required timeframes
- Maintain confidentiality and professional boundaries
- Utilize trauma-informed, recovery-oriented, and person-centered approaches
- Transport members when appropriate to support treatment goals
Qualifications
Applicants must possess:
- Bachelor's or Master's degree in Human Services or a related field, including:
- Social Work
- Psychology
- Counseling
- Marriage & Family Therapy
- Education
- Criminal Justice
- Nursing
- Occupational Therapy
- Recreational Therapy
- Sociology
- Human Development
- Or another qualifying human services field
Preferred qualifications include:
- Experience working with individuals experiencing mental health and/or substance use disorders
- Strong communication and documentation skills
- Excellent organization and time management
- Ability to work independently while collaborating within a multidisciplinary team
- Compassion, empathy, and professionalism
- High emotional intelligence and problem-solving skills
Applicants must also:
- Have a valid driver's license
- Maintain reliable transportation and automobile insurance
- Be comfortable providing services in homes and community settings
- Successfully complete all required onboarding, background checks, and CCS training
Schedule
- Full-Time and Part-Time positions available
- Flexible scheduling
- Daytime, evening, and weekend availability
- Providers create and manage their own schedules
- Opportunity to build and maintain a consistent caseload
Pay: $50.00 - $70.00 per hour
