HABILITATION COORDINATOR
The Habilitation Coordinator is responsible to address and coordinate a wide variety of issues related to service planning, service delivery and special challenges through a person-centered approach, working for the satisfaction of the individual with their involvement in Adult Services. Personal development leading to community inclusion for individuals served is the framework that drives performance of responsibilities and duties in this position.
ESSENTIAL DUTIES:
· Develop, coordinate and monitor support including, but not limited to, adult day services, HPC services and participation in recreational and social activities.
· Provide training and technical assistance for all direct care staff regarding goals, outcomes, services and support for all clients.
· Communicate with SSA’s for the purpose of coordinating services and ensure that these services are being provided in accordance with the Individual Service Plan.
· Monitor MUI’s and UI’s and take immediate action to ensure health, safety and welfare of the program’s clients
· Participate in team meetings as appropriate, including, but not limited to, annual meetings, quarterly meetings, special team meetings and plan revision meetings
· Communicate issues and provide updates regarding clients in a timely and effective manner
· Serve as a “mentor” to provide guidance to new direct support employees regarding techniques and practices that enhance the effectiveness of provision of supports
· Keeps clear, concise, and accurate documentation and other records regarding clients
· Ensure and maintain effective relationships with other employees, individuals, parents/guardians, and state and local community service agencies
· May be required to temporarily cover a unit or drive a transportation route, if necessary.
· Adheres to all state, federal, local and company policies
· Other duties as assigned
QUALIFICATIONS and CHARACTERISTICS:
· High school diploma or GED; bachelor’s degree in social work or other discipline preferred
· 2 years of experience as direct support professional preferred
· Warm, caring demeanor
· Excellent communication skills; ability to train staff
· Able to maintain confidentiality and understand/follow HIPAA laws
PHYSICAL REQUIREMENTS: Must be able to stand, walk, lift, and move objects. Must be able to stoop, stretch, bend, and lift up to 50 pounds. Must be able to push clients in wheelchairs if necessary. May be required to physically restrain client if behavior is a danger to themselves or others. Must be able to sit to use computer and telephone. Eyesight capabilities must be sufficient to perform these functions. Hearing must be adequate to perform these functions.
OTHER REQUIREMENTS:
· Valid driver’s license
· Pass criminal background check
· Pass pre-employment drug screen
· Pass pre-employment physical exam
· Complete first aid and AED certification
· Complete all DODD training requirements
Pay: From $19.00 per hour
