Physical Therapist (PT) – Full-Time

Ultimate Therapy is seeking a Physical Therapist (PT) to join our skilled nursing and rehabilitation team. This role offers the opportunity to provide individualized therapy services that help residents improve mobility, functional performance, balance, and overall quality of life.
What You'll Do:
- Perform comprehensive physical therapy evaluations and establish treatment goals based on resident needs
- Provide therapy interventions focused on gait training, strengthening, endurance, balance, transfer training, and pain reduction
- Work closely with PTA staff and interdisciplinary team members to support continuity of care
- Monitor resident progress and modify treatment approaches as appropriate
- Maintain timely and compliant documentation
