Responsibilities
Provides pharmaceutical care services, training and support to the staff and physicians of UF Health Central Florida focused glycemic management and HCHAP Medication Scores. Is responsible for glycemic management including the optimal use of hyperglycemic medications and improving medication-related HCHAPS scores. Is involved in various activities that include, but not limited to Participation on Quality Improvement Teams, Focus on Core Quality Measures, Drug Use Evaluations. Attendance and involvement in varied medical staff committees/meetings; adverse drug reaction reporting and follow-up, formulary maintenance, one on one physician engagement. The focus of this position will be to train and develop pharmacists and physicians to provide evidenced care directly to patients.

Qualifications
- Education
- Doctor of Pharmacy (Pharm.D.)
- Post Graduate Training
- PGY1 or equivalent experience as clinical pharmacist
- PGY2 in specialty or equivalent experience
- Licensure/Certification/Registration
- Valid Florida Pharmacist License
- Maintain Board Certification in specialty from the Board of Pharmacy Specialties (BPS) or equivalent certification if not offered by BPS
- Special Skills/Qualifications/Additional Training/Experience Required
- Must be able to read, write, speak and understand English.
- Attend at least one nationally recognized meeting in area of specialty each year
- Maintain current membership in organization dedicated to area of specialty
