Self-Directed Assistant/ HBS Case Manager
Full-time Hourly $22.30- $25.80

We are seeking a compassionate and organized Self-Directed Assistant (SDA) / HBS Case Manager to support individuals and families enrolled in the Home-Based Services (HBS) Program. In this role, you will help clients maintain independence by coordinating services, managing budgets, and advocating for their needs while ensuring compliance with state and federal guidelines.
Key Responsibilities:
- Provide advocacy and service coordination for individuals with developmental disabilities
- Assist clients and families with budget management and service planning
- Conduct regular check-ins and in-person home visits to assess needs and progress
- Coordinate care with medical, educational, and community providers
- Maintain accurate documentation, reports, and case notes
- Ensure clients receive appropriate benefits and support services
- Promote independence, community integration, and overall well-being
Qualifications:
- Bachelor's degree in Social Work, Psychology, or related field
- At least 1 year of experience working with individuals with developmental disabilities
- Valid driver's license and reliable transportation
- Ability to obtain required certifications (CPR, First Aid, QIDP training)
- Strong communication, organizational, and problem-solving skills
Work Schedule:
Monday–Friday 9 AM-5 PM with occasional evenings based on client needs.
Salary : $22 - $26
