Join our compassionate and collaborative care team supporting the Senior Care program! As an RN, you will deliver high-quality, patient-centered care for our senior population. We are seeking a nurse who brings compassion, professionalism, clinical expertise, and a commitment to excellence in geriatric care. Responsibilities include, but are not limited to:
- Provide direct patient care, including assessments, medication administration, etc
- Monitor and document patients’ clinical status and communicate changes to the care team
- Triage phone calls and respond to patient concerns, questions, and follow-up needs
- Provide age-appropriate education to patients and families on disease processes, medications, and wellness strategies
- Participate in care team huddles, case management discussions, and interdisciplinary meetings
- Support care coordination, referrals, and transitions of care for patients moving between clinic, hospital, and home
- Engage in quality improvement initiatives, infection prevention, and population health management projects
- Provide mentorship to new clinical staff
Qualifications
Required:
- At least 6 months of experience as an RN
- Bachelor or Master of Science in Nursing
- Current valid California Registered Nurse license
- BLS from the American Red Cross or American Heart Association
- Ability to complete physical assessments
- Skill in developing a plan of care including setting goals, monitoring and documenting progress
- Ability to implement interventions that are pertinent to goals established in the plan of care
- Ability to evaluate the effective of interventions toward the attainment of established goals and to recognize the need to reassess or reset goal parameters
- Ability to delegate tasks in a management style that is consistent with promoting leadership and teamwork
- Knowledge base to provide education to patients, families, and other staff regarding clinical and patient care issues
