The Semel Institute for Neuroscience and Human Behavior is seeking a compassionate and highly organized part-time Care Coordinator to support TIES for Families, a nationally recognized interdisciplinary program dedicated to improving outcomes for children with complex needs adopted from foster care. Working closely with the Director of Clinical Services and a multidisciplinary team, you’ll play a key role in coordinating care, supporting families, and ensure services are delivered seamlessly.
Primary responsibilities include:
· Coordinate services and referrals for children and families, connecting them with internal and community-based resources
· Serve as a point of contact for families, providing guidance, support, and advocacy
· Collaborate with clinicians, the Department of Children and Family Services (DCFS), and community parters to support care plans
· Schedule and organize appointments, case conferences, and team meetings
· Track case activity, maintain accurate documentation, and support billing processes
· Contribute to data collection, reporting, and program operations
· Support special projects and help improve systems that enhance care coordination

Hourly range: $30.36-$43.49
