Introduction:
Family Ties Home Care has a client in Agoura Hills who needs help two days a week. He has Dementia but is in good physical shape, but experiences some dizziness and needs stand by assist. Lives with his wife who needs help while she is out at work. He enjoys cooking. Mostly companion and supervision shift

Responsibilities:
- Following a pre-set care plan
- Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
- Assist with medication reminders
- Monitor and report on client's well-being
- Light housekeeping, laundry, & other household daily chores as needed
- Grocery shopping & errands
- Cooking/meal preparation
- Scheduling appointments, including doctor's
- Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status
- Daily notes
- Other duties assigned as needed
Qualifications:
- Current HCA or higher certification
- Caregiving experience of 1 year or similar experience
- Compassionate and caring demeanor
- Excellent communication & interpersonal skills
- Ability to follow the pre-set care plan and medication schedule
- Ability to lift 35 pounds
- Ability to assist with client's mobility as needed
- Valid driver's license and reliable transportation
- Ability to work the schedule as described
