UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.

Essential Job Functions
- Administers medications and treatments as prescribed by the physician.
- Monitors patient response to interventions.
- When providing home infusion skilled nursing services, they will assess and monitor patients for anaphylaxis and intervene as needed.
- Successfully instructs patients on home infusion therapy self- administration, provide direct nursing care in patient’s home for the administration of home infusion therapies, and troubleshoot patient’s infusion concerns.
- Regularly re-evaluates patient nursing needs.
- Administers medications and treatments as prescribed by the physician.
- Monitors patient response to interventions.
- When providing home infusion skilled nursing services, they will assess and monitor patients for anaphylaxis and intervene as needed.
- Successfully instructs patients on home infusion therapy self- administration, provide direct nursing care in patient’s home for the administration of home infusion therapies, and troubleshoot patient’s infusion concerns.
- Regularly re-evaluates patient nursing needs.
- Completes an initial assessment of patient and family to determine home care needs.
- Provides a complete physical assessment and history of current and previous illness(es).
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Develops a care plan which establishes goals, and incorporates therapeutic, preventive and nursing actions, as well as psychosocial support as needed. Includes the patient and the family in the planning process.
- Complete all required patient care documentation, nursing plan of care and reports.
- Other Duties as Assigned
- One to two years of acute care experience in an institutional setting.
- Possesses and maintains a valid driver’s license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits.
- Current state of Illinois nursing license
- BLS
- ACLS
- Chemotherapy & Immunotherapy Class – Adult
- Hematology Oncology Bridging Course; Fundamentals ONS
- Chemotherapy/ Immunotherapy Certificate within 12 months of hire.
- One year of home health care experience
- Oncology experience
- Infusion experience
