Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of EDRP application. Former EDRP participants ineligible to apply.

Duties

  • The primary purpose of the nurse practitioner (NP) is to practice to the full extent of their education - training - and certification as a licensed independent practitioner to provide healthcare to individuals - families - and/or groups in a variety of settings to include virtual modalities
  • Primary Care Core Clinical Privileges will guide advanced practice beyond basic education and are renewed every two years
  • The NP has prescriptive authority within the program
  • The NP in Home Based Primary Care (HBPC) collaborates within a Patient Aligned Care Team (PACT) to provide clinical care to patients through an outpatient - home-based system
  • Other duties include and not limited to the following: Functions as the case manager for a complex Veteran population - providing hands-on and virtual patient care - and assumes primary medical responsibility for the patients assigned to their PACT team
  • Functions within an interdisciplinary team that delivers care in the home to Veterans who are unable to come to the medical center for routine clinic appointments
  • Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others and proficiency in decision-making and using the nursing process for complex situations that effect performance outcomes at the point of care
  • Assumes primary medical responsibility for the HBPC patients in their case load - identifying medical problems and defining their medical management
  • Evaluates - consults - develops diagnosis - and manages the HBPC panel of patients assigned
  • Anticipates and recognizes medical/surgical/psychiatric emergencies and initiates or leads others to implement appropriate and timely actions
  • Enters progress notes in computerized medical records and completes workload documentation in a timely manner
  • Provides medical care encompassing acute and chronic medical conditions and all appropriate and preventative care for both male and female Veterans
  • Orders diagnostic tests - medications - laboratory tests as medically relevant for each patient
  • Provides timely follow-ups on results - including completion of view alerts
  • Determines medical assessment and develops treatment plans according to accepted protocols
  • Prescribes medications according to approved formulary - and in accordance with Federal and State Law
  • Educates patients concerning medical conditions and recommended treatments such as nutritional counseling - need for smoking - cessation - need for periodic immunizations and health screening tests and other preventive health measures
  • Consults other specialties as appropriate - according to service agreements
  • Leads and organizes delivery-of-care to assure continuity-of-care - including access-to- care and discharge planning
  • Guides staff in the application of critical thinking and process improvement to improve patient care
  • Demonstrates patient advocacy to protect the health - safety - and rights of patients
  • Initiates/participates on interdisciplinary committees - task forces - and groups to improve patient care at the unit- - team- - work group- - program- - or VISN level
  • Demonstrates positive - effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers
  • Displays courtesy - empathy - tact - and respect for patients - families - caregivers - significant others - and staff
  • VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary - regular salary increases - potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave - 13 days of sick leave - 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: Monday - Friday - 8:00am - 4:30pm - subject to change based on the needs of the facility
