Advanced Practice Nurse (CNP)

Summary

This position is eligible for Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• Major Duties include - but are not limited to: Providing advanced clinical practice - consultation - and management
• Practicing to the full extent of their education - training - and certification
• Independently managing a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Developing and implementing an individualized plan to achieve optimal population health and disease management
• Demonstrating leadership and working collaboratively with Primary Care Team and other interdisciplinary team members
• Ordering and interpreting labs - tests - and diagnostic studies
• Prescribing pharmacologic and non-pharmacologic interventions
• Initiating referrals and consults to specialty services
• Providing education to patients and their families
• Assessing and adjusting the plan of care as needed to achieve optimal patient outcomes
