Overview
We are seeking a dynamic and experienced Director of Patient Services to lead our clinical operations and ensure the delivery of exceptional patient care across various long-term care settings. The ideal candidate will possess extensive knowledge of nursing administration, regulatory compliance, and care management, with a passion for fostering a patient-centered environment that promotes safety, dignity, and quality outcomes.
Duties
- Conduct client assessment/reassessment.
- Create and implement care plans tailored to clients' individual needs.
- Conduct supervisory visits to ensure aide performance meets standards.
- Conduct orientation and in-service training.
- Lead and supervise nursing staff and care teams, ensuring effective shift management, staff training, and performance evaluations.
- Ensure compliance with all federal, state, and local regulations and other regulatory requirements.
- Maintain physician orders, care plans and service authorizations.
- Collaborate with interdisciplinary teams to optimize outcomes through quality improvement initiatives and evidence-based practices.
- Monitor operational metrics related to patient care quality, safety incidents, and regulatory audits; implement corrective actions as needed.
- Investigate complaints.
- Foster a culture of leadership, accountability, and continuous improvement among staff members while promoting client satisfaction.
Skills
- Homecare experience with a strong understanding of regulatory compliance and long-term care regulations.
- Proven management skills with supervising experience in clinical or administrative roles within healthcare settings.
- Experience with EHR systems for managing records and documentation efficiently.
- Experience in nursing leadership, care plans development, and managing care teams.
- Familiarity with Medicare regulations and home health agency operations.
- Excellent leadership abilities combined with effective communication skills.
Qualifications
- Current New York State RN License.
- Graduate of accredited school of nursing.
- Minimum of one year of homecare experience.
- Knowledge of NYS Licensed Home Care Services Agency regulations.
- Experience with EHR systems for managing records and documentation efficiently.
