What are the responsibilities and job description for the LPN (Licensed Practical Nurse) position at U.P. Home Health & Hospice?
As an appreciated & valued Licensed Practical Nurse with our organization, you will uphold the company's mission of offering high-quality care and services with compassion, dignity and respect. The successful candidate in this position will help meet our patients' home health nursing needs, including administering injections and intravenous medications, monitoring vital signs, and providing important skilled care. You will work directly under the supervision of a Registered Nurse and other licensed medical professionals. Licensed Practical Nurses will travel throughout Marquette County to provide home visits to provide medically ordered skilled care.
Responsibilities:
- Facilitate the delivery of skilled patient care in a variety of health care settings, including private homes.
- Monitor and document vital signs.
- Monitor and assess patient conditions and response to treatment.
- Administer medications, treatments, and personal care to patients under the supervision of a registered nurse.
- Evaluate the needs of patients in order to coordinate care with other medical professionals.
- Gather and document patient information.
