Position Summary:
We are seeking a Nurse Practitioner (AGNP or CNP) to provide on-site primary care and wound treatment services to residents in assisted living and memory care communities. If you don’t already have wound care certification, we’ll fully cover the cost so you can expand your skillset and make an even greater impact.
Key Responsibilities:
- Travel to partner communities to deliver bedside primary and wound care
- Manage chronic illnesses, preventive screenings, and advanced care planning
- Collaborate with interdisciplinary teams and maintain consistent communication
- Document clinical encounters and update care plans as needed
Qualifications:
- Certified Nurse Practitioner licensed in Minnesota
- AGNP certification preferred
- Experience in geriatrics, home-based care, or internal medicine
- Valid driver’s license and auto insurance required
- Excellent interpersonal and time management skills
