What are the responsibilities and job description for the Primary Care Physician (MD) – Rounding Geriatric Primary Care position at Twin Cities Physicians?
Job Title: Primary Care Physician (MD) – Geriatric Medicine
Company: Twin Cities Physicians
Location: Greater Twin Cities Metro Area
Schedule: Full-Time | Weekday-Focused Rounding Model
Compensation: $250,000–$325,000 Annually
Be the Physician Other Clinicians Turn To
There are physicians who see patients.
And there are physicians people look for when the case gets complicated.
The patient has five chronic conditions. Medications are interacting. Something has changed since the hospitalization. The nurse is concerned. The family has questions. An NP or PA wants another clinical perspective.
That's where you come in.
Twin Cities Physicians is looking for an experienced Primary Care Physician to care for medically complex older adults throughout senior living communities across the Twin Cities.
You'll practice medicine, manage your own patients, and develop meaningful longitudinal relationships.
But your impact goes beyond your own patient panel.
You'll also work alongside Nurse Practitioners, Physician Assistants, nurses, and senior living clinical teams, bringing the experience, judgment, and physician leadership that strengthens the entire care model.
Keep practicing medicine. Expand the number of people your expertise influences.
Your Experience Should Matter Beyond Your Own Schedule
After years of practicing medicine, you've developed something that can't be learned from a textbook.
Judgment.
You know when something doesn't look right.
You know when another test probably won't change the treatment plan.
You understand how diabetes, CHF, COPD, dementia, polypharmacy, frailty, functional decline, and family dynamics can collide in the same patient.
And you've learned that sometimes the most important clinical decision is determining what not to do next.
Geriatric primary care puts that judgment to work.
You'll care for older adults in assisted living, memory care, independent living, and long-term care communities, where complex clinical decisions are part of everyday medicine.
This is medicine that rewards experience.
Be a Physician and a Clinical Resource
Twin Cities Physicians utilizes a collaborative provider model.
Our NPs and PAs have meaningful autonomy, but they also benefit from having experienced physicians available when a patient becomes particularly complex or another perspective is needed.
You'll help create that clinical backbone.
That may mean discussing a difficult case with an APP, collaborating with a community nursing team, navigating a complicated transition from the hospital, speaking with a family, or determining the appropriate next step when a patient's condition changes.
You're not leaving patient care for administration.
You're using your clinical experience to make the entire team stronger.
What Your Practice Looks Like
Rather than spending every day inside the same clinic, you'll conduct on-site rounds within senior living communities throughout the Greater Twin Cities.
You'll:
- Provide comprehensive primary care to medically complex older adults
- Diagnose and manage acute and chronic medical conditions
- Build an ongoing patient population and follow patients longitudinally
- Manage diabetes, CHF, COPD, hypertension, dementia, polypharmacy, and other complex conditions
- Provide preventative care and chronic disease management
- Evaluate meaningful changes in patient condition
- Coordinate care following hospitalizations and specialty consultations
- Collaborate with NPs, PAs, nurses, pharmacists, therapists, and community clinical teams
- Provide physician-level guidance on complex clinical situations
- Participate in patient and family care discussions
- Help identify concerns early and reduce avoidable hospitalizations and readmissions
- Maintain accurate and timely clinical documentation
Complex Medicine Without the Traditional Clinic Grind
Clinical complexity doesn't have to mean an unsustainable physician lifestyle.
This opportunity offers:
- $250,000–$325,000 annual compensation
- Weekday-focused rounding model
- No traditional clinic waiting room
- Longitudinal relationships with patients
- Significant clinical autonomy
- Experienced NP and PA collaboration
- Established clinical and administrative infrastructure
- Opportunity to influence care beyond your own patient panel
- A growing Minnesota patient population
- A physician-led medical group behind your practice
Your workday will involve travel between assigned communities rather than sitting in the same clinic.
For the right physician, that's part of what makes the model appealing.
You go where your patients are.
Who We're Looking For
You may come from Family Medicine, Internal Medicine, Geriatrics, primary care, post-acute medicine, skilled nursing, hospital medicine, or another environment serving medically complex adults.
We're looking for a physician who brings:
- MD or DO
- Active Minnesota physician license in good standing
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Strong clinical judgment with medically complex patients
- Confidence managing multiple chronic conditions and polypharmacy
- Ability to collaborate effectively with Advanced Practice Providers
- Strong communication with patients, families, and interdisciplinary teams
- Genuine interest in caring for older adults
- Valid driver's license and reliable transportation for community-based practice
Direct senior living experience is valuable, but it isn't the only path here.
We're more interested in whether you've developed the clinical judgment this patient population deserves.
4,500 Seniors. 350 Communities. One Growing Medical Group.
Twin Cities Physicians is a physician-led medical group providing on-site primary care to more than 4,500 older adults across 350 senior living communities throughout Minnesota.
Our providers bring medical care directly into the places seniors call home, helping improve access, continuity, communication, care coordination, and outcomes while working to reduce avoidable hospitalizations.
We're continuing to grow, and that growth requires more than additional providers.
It requires experienced physicians who can help set the clinical standard.
Your Next Chapter Doesn't Have to Take You Away From Medicine
For many experienced physicians, career advancement eventually seems to mean seeing fewer patients and attending more meetings.
It doesn't have to.
Here, you can continue diagnosing, treating, managing complex disease, and developing patient relationships while also becoming a physician resource for the clinicians practicing alongside you.
Your experience gets multiplied.
Your judgment influences more than your own patient panel.
And you can still finish the day knowing you practiced medicine.
If you're looking for a physician role where your clinical experience can have a broader impact while earning $250,000–$325,000, we'd like to meet you.
