What are the responsibilities and job description for the Physician Assistant (PA-C) – Long-Term Care Primary Care position at Twin Cities Physicians?
Job Title: Physician Assistant (PA-C) – Geriatric Primary Care
Company: Twin Cities Physicians
Location: Southern Minnesota | Rochester, Owatonna, Albert Lea & Surrounding Communities
Schedule: Full-Time or Part-Time | Monday–Friday | No Evenings, Weekends, or Hospital Call
Compensation: $120,000–$160,000 Total Compensation (Base Salary Productivity Incentive Program) Mileage Reimbursement
What If Your Next Patient Needed You to Think Like a Generalist Again?
Not every patient fits neatly into one diagnosis.
Especially not in geriatric medicine.
Diabetes. Heart failure. COPD. Dementia. Hypertension. Polypharmacy. A recent hospitalization. A new symptom that could mean five different things.
You have to connect the dots.
Twin Cities Physicians is hiring a Physician Assistant to provide on-site primary care to older adults throughout Rochester, Owatonna, Albert Lea, and surrounding Southern Minnesota communities.
This is an opportunity to use the breadth of your PA training while managing medically complex patients over time.
Not one body system. Not one episode of care. The whole patient.
Remember Why the PA Profession Appealed to You?
You were trained broadly.
To assess. Diagnose. Treat. Adapt. Collaborate. Solve problems.
But careers can gradually become narrower.
You specialize. Your patient population becomes more defined. Your appointments become more standardized. Eventually, you may find yourself using only a fraction of what you learned.
Geriatric primary care is different.
One visit may involve adjusting diabetes medications. Another requires evaluating new confusion. You're monitoring CHF, reviewing medications after a hospitalization, discussing goals with a family, or determining whether a subtle change requires immediate intervention.
Your clinical judgment gets used here.
Complex Patients. Real Clinical Ownership.
You'll manage an ongoing panel of older adults living in assisted living, memory care, independent living, and skilled nursing communities.
Because you follow patients over time, you're able to see how the pieces fit together.
You know their baseline.
You know what changed after the last hospitalization.
You understand what medications they're taking and why.
And you can communicate directly with the nurses and caregivers who see them every day.
You'll have meaningful autonomy to assess, diagnose, treat, prescribe, manage chronic disease, and develop individualized treatment plans, with an established physician-led medical group behind you.
What You'll Be Doing
- Provide comprehensive on-site primary care within senior living communities
- Travel throughout Rochester, Owatonna, Albert Lea, and surrounding areas
- Manage an ongoing panel of older adult and medically complex patients
- Diagnose and treat acute and chronic conditions
- Complete assessments, annual wellness visits, preventative care, and follow-ups
- Manage multiple chronic diseases and medications
- Evaluate new symptoms and changes in condition
- Coordinate care following hospitalizations and specialty visits
- Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and families
- Help identify concerns early and reduce avoidable hospitalizations and readmissions
- Maintain accurate and timely clinical documentation
Bring Us the Experience You've Built
You don't need senior living experience.
In fact, your experience in another clinical environment may be exactly what makes you successful here.
We're interested in PAs coming from family medicine, primary care, internal medicine, urgent care, hospital medicine, geriatrics, post-acute care, skilled nursing, home health, hospice/palliative care, and other settings serving medically complex adults.
We're especially interested in someone who enjoys the patient who makes you stop, think, investigate, and determine what is actually going on.
You should bring:
- Graduate of an accredited Physician Assistant program
- Active Minnesota Physician Assistant license
- Strong assessment, diagnostic, and clinical decision-making skills
- Confidence managing patients with multiple chronic conditions
- Strong medication management and care coordination skills
- Comfort working with significant autonomy while collaborating with physicians
- Genuine interest in older adults and complex medicine
- Reliable transportation and willingness to travel throughout Southern Minnesota
Experience in geriatrics, chronic disease management, senior living, or community-based medicine is highly valued. Wound Care Certification (WCC) is a plus.
A Career That Uses More of What You Know
Clinical challenge doesn't have to mean sacrificing your life outside of work.
This opportunity includes:
- $120,000–$160,000 total compensation
- Base salary productivity incentive program
- Full-Time or Part-Time
- Monday through Friday
- No evenings, weekends, or hospital call
- Mileage reimbursement
- Significant clinical autonomy
- Your own ongoing patient panel
- Collaboration with a physician-led clinical team
- Comprehensive medical and dental benefits
- PTO paid holidays
- CME allowance
- Malpractice coverage
- DEA reimbursement
- Clinical and administrative resources
Your Patients Won't Always Be Simple. Your Schedule Can Be.
You'll travel between senior living communities rather than reporting to the same clinic each morning.
But when your workday is finished, you're not transitioning into evening clinic or preparing for weekend hospital call.
Monday through Friday. No evenings. No weekends. No hospital call.
It's an opportunity to take on clinically interesting, medically complex patients without automatically accepting an unpredictable provider lifestyle.
