Job Title: AGNP / CNP – Field-Based Senior Care | Wound Training Independent Role

Responsibilities:
- Serve as the go-to provider in your assigned communities
- Deliver preventative, acute, and wound care services
- Collaborate with care teams and families
- Maintain up-to-date medical documentation
Qualifications:
- Minnesota Nurse Practitioner license (AGNP or CNP)
- Wound care certification or willingness to obtain it (we cover the cost!)
- Excellent time management and bedside manner
- Experience with geriatrics or long-term care a plus
