Overview
The Tuscola County Health Department is seeking a Social Worker for a specially funded/contractual position with flexible scheduling, working up to 15 hours per week, (not to exceed 999 hours worked per year). This position offers the opportunity to create a work schedule that aligns with both program needs and the availability of the employee. This position works in the Maternal Child Health Program under the supervision of the Nursing Administrator and provides services to individuals and families in the office, home, and/or community environment. Maternal Infant Health Program (MIHP) is the largest home visiting program in Michigan. MIHP provides home visits and care coordination to Medicaid eligible women and infants. This position will be supervised by a LMSW.
Duties
- Provide home visits and care coordination.
- Promote healthy pregnancies, positive birth outcomes and healthy infant growth and development.
- Work to improve the health and wellbeing of Medicaid eligible pregnant women and infants through an evidence-based approach.
- Collaborate and work in cooperation with patient's physician, public health nurse, therapists and other health care providers.
Requirements
- Must possess a Bachelor's degree from a school accredited by the Council of Social Work Education and at least one year experience providing Social Work services to families. Master's degree preferred.
- Must possess current licensure as a licensed social worker by the State of Michigan Licensing and Regulatory Affairs.
- Must possess a valid Michigan driver's license without limitations.
- A background check and drug screening will be required.

Pay: $37.00 - $39.00 per hour
