Requirements / Qualifications
High school Diploma Immunizations: TDAP (within the last 10 years), MMR (2 within a lifetime)
- Driver's License Copy (Current Drivers License and Proof of liability inusrance)
- Proof of HS Graduation (Copy of High School Diploma or GED)
