Position Title: Medical Social Worker (MSW)

Location: San Francisco, CA 94110

Duration: 13 Weeks with possible extension

Any pick any work schedule

• Mon to Thu: 7am -5:30pm (10 hr. shift), 40 Hours/week at Renal Center
• Mon to Fri: 8:30am - 5pm (8 hr. shift), 40 Hours/week at Inpatient Psychiatry
• 8:30am - 5pm (8 hr. shift), 40 hrs./week any 5 days in the week weekend flexibility is must, FLOAT - Emergency Dept and Inpatient Med Surge Areas ,

Job Duties:

• Conduct diagnostic evaluations to assess client needs and develop appropriate treatment plans.
• Oversee the implementation of home community care services, ensuring adherence to best practices and regulatory requirements.
• Ensure compliance with HIPAA regulations to protect client confidentiality and privacy.
• Collaborate with interdisciplinary teams to coordinate behavioural health services and support client progress.
• Monitor staff performance, providing feedback and conducting performance evaluations.
• Participate in ongoing professional development to stay current with industry trends and best practices.

Requirements:

• Possession of a master’s degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE).
• Must be registered with board of Behavioural Sciences
• Experience as an MSW.
• At least 1-2 Years of Experience as an MSW in facilities in a large urban area, Acute Trauma Hospital.
• Previous hands-on experience in executing all typical tasks & deliverables & in the entire lifecycle
• Assessments, counselling & care coordination , resource & benefit acquisition, and discharge planning.
• Health education.
• Documentation.
• Patient load metric of 15-20 assessments.
