As the Nurse Manager/Health Services Director at Trustwell Living, you'll be the driving force behind exceptional care for our Assisted Living residents. From ensuring safe medication management and infection control to mentoring staff and coordinating personalized care plans, you’ll help create a thriving, supportive environment where seniors can live their best lives.
The successful candidate for this position will have:
- A passion for seniors and enhancing their quality of life
- A valid Registered Nurse (RN) license; a Compact license or the ability to obtain one is encouraged. An unencumbered license is required. An LPN with leadership experience may also be considered.
- Proven clinical leader in an assisted living or LTC environment with at least one year of successful experience as a Wellness Director, Health Care Coordinator (HCC) or Director of Nursing (DON)
- Understand the importance of being state survey-ready, every day
- Have a current driver’s license
Essential Functions:
- Be able to lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals.
- Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers and pharmacy staff
- Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process
- Visit prospective/current residents in their homes or healthcare settings as needed to assess for community admission or readmission.
- Clinical liaison with families, residents, healthcare professionals, and team members
- Direct the infection control measures for the community
- Work collaboratively with others on the community leadership team
- Manage and coordinate the work of the clinical staff. This includes overseeing and/or completing required training, staff schedules, and monthly and daily work assignments.
- Provide on-call clinical support as needed.
