As the Assistant Health Services Director at Trustwell Living Eagle Point Place, you'll be the driving force behind exceptional care for our Assisted Living residents. From ensuring safe medication management and infection control to mentoring staff and coordinating personalized care plans, you’ll help create a thriving, supportive environment where seniors can live their best lives.

The successful candidate for this position will have:

• A passion for seniors and enhancing their quality of life
• A valid Registered Nurse (RN ) license; a Compact license or the ability to obtain one is encouraged. An unencumbered license is required.
• Proven clinical leader in an assisted living or LTC environment with at least one year of successful experience as a Wellness Director, Health Care Coordinator (HCC), or Director of Nursing (DON)
• Understand the importance of being state survey-ready every day
• Have a current driver’s license

Essential Functions:

• In coordination with the Health Services Director, lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals.
• Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers, and pharmacy staff
• Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process
• Visit prospective and current residents in their homes or healthcare settings as needed to assess their suitability for admission or readmission to the community.
• Clinical liaison with families, residents, healthcare professionals, and team members, and refers them to the Health Services Director when appropriate.
• Monitors the quality of work performance of Care Partners, Medication Staff, Licensed Nurses, and reports to the Health Services Director.
• Ensures that infection control procedures are known and followed by staff. Directs the infection control measures for the community.
• Works closely with staff and designated agent(s) to identify Resident problems, concerns, and issues; follows up as needed and communicates the findings appropriately.
• Oversees and/or completes monthly nursing schedule for nursing staff. Participates in the Manager on Duty program as directed by the Executive Director.
• Ensures the accurate transcription of physician orders into the treatment and medication record.
• Assists as directed with move-in and orientation of new Residents and designated agent(s).
• Reviews Resident service records, treatment, and medication books to assure quality of care and documentation in the medical record is within nursing standards.
• Verifies physician orders, move-in/move-out information, and initiates appropriate action/follow-up.
• Supervises the ordering of medications from the pharmacy.
