Clinic Nurse Practitioner
We're seeking a skilled Clinic Nurse Practitioner to join our team at TruHealth. As a key member of our care team, you'll play a vital role in promoting dignity, independence, and quality of life for our seniors.
Key Responsibilities:
- Assess patients' medical and mental health needs and provide a plan of care that proactively manages their conditions and addresses barriers to care
- Ensure compliance with local, state, and federal agencies related to clinical services you provide
- Monitor patients' compliance and response to their treatment and modify those plans as needed
- Work with RN case managers as part of an integrated care team
Requirements:
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook, and Excel
Salary : $80,000 - $120,000
