Job Overview
True North Clinical Services, LLC is seeking dedicated and compassionate Intensive In-Community (IIC) Agency staff to provide comprehensive mental health support within community settings. This role involves working closely with individuals experiencing mental health challenges, offering personalized care, and promoting recovery and independence. The position is ideal for professionals committed to making a positive impact in clients' lives through community-based services.
Duties
- Deliver individualized mental health interventions in clients' homes and community environments
- Develop and implement personalized treatment plans in collaboration with clients, families, and interdisciplinary teams
- Assist clients with daily living skills, medication management, and social integration
- Monitor client progress and adjust care strategies accordingly
- Document all interactions accurately in compliance with agency policies and regulatory standards
- Provide crisis intervention as needed to ensure client safety
- Collaborate with healthcare providers, case managers, and support networks to coordinate care
- Educate clients and families about mental health conditions and available resources
Requirements:
Active NJ license (LCSW, LPC, LMFT, LSW, LAC, LAMFT)Experience in psychotherapy with children
Valid driver's license & reliable vehicle
Professional liability insurance
