This role ensures appropriate nurse placement based on patient needs, physician orders, and nurse availability. The Coordinator serves as a central point of communication between patients, nurses, physicians, and internal staff to ensure continuity of care, compliance, and quality service delivery.Responsibilities:Coordinate and schedule Field Nurses based on patient care needs, active physician orders, and nurse availabilityMaintain ongoing communication with nurses regarding assignments, visit confirmations, attendance, and schedule changesEnsure all physician orders and care plans are active and accurately documented prior to scheduled visitsManage nurse schedules using HHA eXchange, including monthly service calendars and real-time updatesConfirm completed visits and process time-sensitive field paperwork for weekly payroll submissionMonitor nurse attendance, missed visits, late visits, and call-outs; arrange coverage as neededServe as a liaison between patients, families, nurses, and physicians to support quality home care servicesAnswer incoming phone calls and communicate with patients, families, physicians, and staff in a professional and reassuring mannerMaintain accurate documentation while ensuring patient confidentiality and HIPAA complianceCommunicate staffing challenges, scheduling concerns, and difficult-to-staff cases with the Director and internal teamParticipate in on-call rotation with other scheduling team membersRequirements:High School Diploma or equivalent (required)HHA eXchange experience (preferable)Fluency in Spanish requiredPrior experience in home care, healthcare scheduling, or care coordination preferredStrong verbal and written communication skillsExcellent organizational skills with attention to detailAbility to prioritize tasks and manage time effectively in a fast-paced environmentProfessional, positive, and team-oriented attitudeStrong customer service skills with a patient-focused approachBenefits:Health, dental, and vision insurance401(k) with company matchPaid Vacation, Company Holidays, Sick Leave, and Floating HolidaysProfessional development opportunitiesMission-driven work in an innovative healthcare space
