We are seeking a passionate licensed Social Worker (MSW) to join our team in a per-diem capacity. The Social Worker works alongside a team of professionals (RNs, PTs, OTs, STs,), directing and coordinating care for patients. This is a great position for someone who already works full or part time and feels called to make a meaningful difference in the lives of our aging community members. What You’ll Need to Succeed:
- A genuine motivation to provide high-quality, compassionate care while supporting patient goals.
- Strong relationship-building skills with patients, caregivers, and community partners.
- Graduate of an accredited university with an MSW. Must be fully licensed LICSW in the State of MN.
- A valid driver’s license, and reliable, insured transportation.
- Solid computer literacy and comfort navigating electronic charting systems.
- Excellent clinical assessment, documentation, written, and verbal communication skills.
- Experience connecting individuals with community resources—particularly those supporting older adults—is strongly preferred.
- Ability to work independently, manage time well, and adapt to a dynamic home-care environment.
Position Details:
- This role provides in-home social work visits for patients across our Twin Cities service area; mileage between patient homes is fully reimbursed.
- Anticipated workload is approximately 10–15 hours per month, depending on patient needs during that period.
- This is a per-visit position. You will be compensated $80 per completed visit, which includes brief chart review, care coordination, and any necessary follow-up.
- You control your schedule. Visits can be arranged at times that work best for both you and the patient—complete flexibility with no required set hours (except mutually agreed upon times for onboarding).
- You will work intentionally with our skilled clinical team to ensure coordinated, person-centered care for every patient you see.
