Your Impactful Role as an RN Case Manager 

In your pivotal role based at our Bemidji office, you will: 

• Manage and Schedule Visits: Enjoy the independence of managing your patient visits, traveling within the community to provide personalized, one-on-one nursing services in patients’ homes. This role allows you to make deep connections with patients, ensuring they receive the utmost care and attention.
• Interdisciplinary Collaboration: As a vital part of our interdisciplinary team, your leadership will enhance our collaborative, supportive environment, ensuring the highest well-being and care standards for our patients.
• Clinical Supervision and Mentorship: Guide and inspire our caregivers to adhere to care plans with dedication, embodying our values of altruism and integrity, which are crucial to the exceptional care we provide.
