What are the responsibilities and job description for the Intake / Patient Care Coordinator/Pharmacy technician position at Trinity Pharmacy Services?
Trinity Pharmacy Services is an Alternate Site Pharmacy that provides Medication Management, Home Infusion, and Sterile and Non-Sterile Compounding services to patients throughout West Virginia, Kentucky and Ohio. We service the chronically ill and patients with specialized needs. Trinity is looking to add an Intake/Patient Care Coordinator to our team. This position entails onboarding new patients, data entry, patient follow-up, communication with referral sources, and integration with our Pharmacy Team. Communication will be via phone, text, email, Microsoft Teams and facsimile. Come join our unique model and be a participant of our exponential growth!
Qualifications:
Certified Pharmacy Technician preferred( entry into our Pharmacy technician trainee program is a possiblity)
