Role Description
We are seeking a compassionate and skilled Registered Home Health Nurse to join our team in SLO North County. This is a part-time role focused on delivering exceptional patient care to individuals in their homes. Responsibilities include providing skilled nursing services such as wound and ostomy care, medication management, safety evaluations, monitoring patients' health conditions, and coordinating care plans with interdisciplinary teams to support patients' recovery and overall well-being.
Qualifications
- Demonstrated skills in Home Care, Patient Care, and delivering tailored healthcare services
- Experience with Wound Care and chronic illness management
- Strong ability to work collaboratively in an interdisciplinary team environment
- Excellent communication and organizational skills
- Current and unrestricted Registered Nurse (RN) license in California
- Experience with home health or senior care services preferred
- Valid driver’s license and reliable transportation
