What You'll Do:
- Conduct comprehensive psychosocial assessments for patients and families.
- Develop treatment plans that address emotional, social, and resource-related needs.
- Provide brief, solution-focused counseling and facilitate discharge or care planning.
- Advocate for patients and coordinate referrals to community-based services.
- Collaborate effectively with interdisciplinary care teams.
- Document all assessments, plans, interventions, and outcomes clearly and promptly.
- Meet departmental productivity expectations and performance standards.
- Education: Master’s Degree in Social Work or a related field from an accredited institution.
- Licensure: Must be eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
- Experience: Minimum of 2 years of social work experience in an acute inpatient setting.
- Strong psychosocial assessment skills and clinical judgment.
- Ability to navigate complex family dynamics and high-acuity situations.
- Excellent crisis support and problem-solving abilities.
- Outstanding interpersonal and empathetic communication skills.
- Proficiency in engaging patients and care teams in effective treatment planning.
- Knowledge of community resources and continuity of care principles.
- Accurate, timely documentation in medical records and reporting systems.
